Private case management services will be delivered by an experienced, tertiary qualified case manager.
The aim of case management is to organise the services based on an individual’s needs, making life easier.

  • Do you feel as if you are getting nowhere in supporting your loved one or your life?
If you answered yes to any of the above than case management may help you.
The goal of case management is to promote and support independence and self-sufficiency. Case Management Support Services offer short term, periodic or longer term services to help you to coordinate a range of services to help you or a loved one to continue to live well in the community. The case manager will assess you and your individual circumstances and needs and recommend appropriate services and goal focused strategies.
All Care Plans are Consumer Directed (CD).
  • you get more say in the care and services you access, how it is delivered and who delivers it to you
  • you will have conversations about your needs and goals
  • you will work in partnership with your service provider to develop your care plan
  • you agree to the level of involvement you will have in managing your care package
  • you will have a greater understanding about how your package is funded and how those funds are spent through your individualised budget and monthly income and expense statement
  • your service provider will monitor and provide you with ongoing formal reviews to ensure that your package still meets your needs.
